Step 3 - Waiver of Liability
Terms of Waiver *
This waiver of liability must be signed by the participant prior to the start of any classes, practice sessions, programs, competitions, or club activities with, or held on the premises of, Capital Fencing Academy. If the participant is a minor (under 18 years of age), the waiver must be completed by a parent or guardian of the participant. No student, participant, or club member will be allowed to participate without this signed waiver. Upon entering events sponsored by or taking place at Capital Fencing Academy, I agree to abide by the rules of Capital Fencing Academy. I understand and appreciate that the participation in a sport carries a risk to me of serious injury, including permanent paralysis, or death. I voluntarily and knowingly recognize, accept and assume this risk and release Capital Fencing Academy, its coaches, its members, and its sponsors (including but not limited to Capital Fencing Academy) from any and all liability stemming from my fencing and related activities.
Step 4 - USFA Registration

In order for any student that participate in any class, lesson or fencing activity at Capital Fencing Academy, the students must also have a current membership with the US Fencing Association (USFA). Capital Fencing Academy is a USFA member club and this is a USFA requirement. Membership for non-competitive fencers for $10 and $70 for competitive fencers.

To complete your fencer's membership application online, please go to: www.usfencing.org and select "Become a Member," and complete the profile information. Select a non-competitive or competitive membership.

All students must bring proof of their membership application to the first day of class in order to participate!!!
Step 5 - Photo Release
Terms of Release *
I understand that Capital Fencing Academy will many use photographs of its students, fencers and families for lawful purposes including publicity, advertising and Web content. I grant to Capital Fencing Academy the right to take photographs of me, my fencer and my family in connection with the classes, lessons and any other Capital Fencing Academy event. I authorize Capital Fencing Academy its assigns and transferees to copyright, use and publish the same in print and/or electronically.
